First Visit

Your initial appointment will consist of a thorough examination and consultation explaining your diagnosis and treatment options. In many cases, our doctors may ask you to return to gather diagnostic impressions and photographs of your teeth in order to fully assess your current dental condition and treatment needs. Our doctors take great care in providing you with the most comprehensive examination possible, evaluating all aspects of your oral health. At each exam visit, you will be screened for several conditions including but not limited to:  periodontal disease, bite and joint problems, cavities, oral cancer or other pathology. We place special emphasis on listening to and respecting our patients main concerns in order to provide you with all available options to achieve optimum oral health.

Please assist us by providing the following information at the time of your consultation:

  • A list of medications you are presently taking.
  • A completed welcome form and medical history form (either mailed to you or printed from our website)
  • Previous xrays and/or pertinent dental records
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit. If parents are unable to be present during treatment visits, we must have a contact number to reach you during your child’s treatment.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy (blood thinners), osteoporosis medications etc.).

X-Rays

If your previous dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our office.
